Forms and Documents that Graduate Students Need
The Graduate Student Handbook contains information regarding the requirements for the MS and PhD degrees. The handbook details the policies and procedures and expectations for graduate students. Also, the handbook provides timelines for fulfilling these requirements.

The Advisory Committee Meeting form should be brought to each meeting of a grad student’s advisory committee, to be completed and signed by the members of the committee and returned to the graduate program director.
The Graduate Student Information form needs to be filled out annually by all graduate students and given to the program director.
